Library Online Request Forms

Many of the services that required New York State Library (NYSL) borrowers to fill out paper forms and submit them in person can now be handled with one of the electronic forms listed below. Note: Some of these forms are limited to NYSL registered borrowers or to other libraries.

Research Library Forms

Form	Use this form to...
Ask a Librarian	Request help with a research or reference question.
Book Purchase Recommendation	Suggest that the New York State Library buy specific items.
Class or Workshop Registration Form	Register for the free classes or workshops provided by the New York State Library.
Interlibrary Loan (ILL) Request for libraries	Place requests for materials held at the State Library. First-time users will need to set up an account first.
Interlibrary Loan (ILL) Request for individuals (borrowers with P-cards or Document Delivery customers)	Log-in with your P-card and place requests for materials not held at the State Library. (First-time users will need to set up an account first.)
Look Up Your PIN or NYSL Borrower's Card Number	Ask us to look up your NYSL Borrower's Card number and/or PIN if you have forgotten it. This service is available only to individuals and libraries that already have a NYSL Borrower's Card.
NOVELny Database Registration Forms	Register for one or more of the NOVELny online databases. This service is available only to libraries in New York State.
Renew NYSL Materials (Individuals)	Request renewals of materials you have borrowed from the New York State Library.
Report an Online Database Problem	Tell us if you cannot access one of the online databases listed on our web site, or if an online database is not working correctly. This form is available only to individuals who have a borrower's card from the New York State Library.

Talking Book and Braille Library (TBBL) Forms

TBBL Form	Use this form to...
Application Forms	Register to borrow recorded or braille books. This form cannot be submitted online. Please print out one of the applications, complete it and mail it back to us.
E-mail Update Form (for individuals)	Update an individual borrower's e-mail address or other contact information.
E-mail Update Form (for institutions)	Update a school's or other institution's e-mail address or contact information.
