E-Rate (Universal Service for Telecommunications Discounts Program) for Libraries

Certification for Not Filing a Library Technology Plan

The _______________________ Library System certifies that we are not required to submit an E-Rate Technology Plan because we will apply for only basic* local and long distance telephone service.

We understand that if our E-Rate application should encompass additional telecommunication services and/or equipment during the current planning cycle, we will be expected to file a Technology Plan for the years remaining in the cycle.

*Basic telephone service is strictly limited to individual local telephone line service, long distance, and mobile services. Local telephone service provided through PBXs, key systems, or Centrex is not considered basic. Higher speed services such as DSL, ISDN, T-1 are also not basic.
