* A library must be a member of a public library system or a reference & research library resources system in order to be eligible for a discount through the Universal Service Program.

*** A certified list of member libraries whose plans have been approved by the system should be sent to Lisa Areford (see above) using the form "Certification of Approval of Member Library Technology Plans for Universal Service Discount Program." Please do not send member library plans to the State Library.
**** Copies of the letters of approval to member libraries will also be sent to the respective systems.
NOTE: Please note that the above table deals only with the process involving technology plans. It does not address the approval of applications for discounts on telecommunications services, which is a separate process that goes directly to the Schools and Libraries Division (SLD) without any involvement by the State Education Department or the New York State Library.
