Certification of Approval of Member Library Technology Plans for E-Rate Discount Program

I certify approval of the technology plans for the E-Rate Discount Program for the attached list of member libraries of the _______________________ Library System. This certification ensures that the technology plans for these libraries meet the five core requirements for these plans with the specific details needed. Copies of these technology plans are available for inspection at system headquarters and the respective member libraries. A copy of each plan will be provided to the New York State Library upon request.

• Name of library
• Mailing address
• Telephone number
• Fax number
• E-mail address
• Library contact person
• Date plan approved by system
• Date plan approval expires
• System contact person including telephone number, fax number, and e-mail address
