The Assurance and Certification forms listed below are required for all LSTA applications for the FY 2008-2010 grant/contract years. The first two certifications are required for all state contracts. The Lobbying, Debarment, Suspension and other Responsibility Matters; and Drug-Free Workplace Requirements and CIPA Certifications are required for federal programs.
The State Library is required to collect Children's Internet Protection Act (CIPA) certifications for all LSTA applications whether your project involves the purchase of computers to access the Internet or any direct costs associated with accessing the Internet. If you are not seeking LSTA funds for either of those purposes, then CIPA does not apply. You would check box B and complete the bottom of the form.
Public libraries and all library systems could be affected by CIPA requirements if you expect to purchase computers to access the Internet or pay for any direct costs associated with accessing the Internet using LSTA funds.
