All of the financial forms required during an LSTA project are available on the New York State Library's web site under the heading "FY2008 LSTA Program." Please be sure that this notice goes to the appropriate staff who handle financial forms.
The Request for Funds Form (FS-25; in Word or .PDF) is a one-page form to be completed quarterly. As indicated on the form, one copy is to be completed and submitted directly to the Grants Finance Unit. The address of this unit is in the upper left hand corner of the form. It is very important that these forms be submitted on a timely basis.
The Budget Amendment Form (FS-10A; in Word or .PDF) is used when funds need to be shifted within a project budget during the course of the grant. A cover letter must accompany the budget amendment form explaining the reasons that an amendment is needed. Send three copies (at least one with original signature) of this form to Mary Linda Todd, Library Development, Room 10B41 CEC, Albany, NY 12230. Changes in personnel and equipment always need prior approval. Provide as much detail as possible in the Explanation column. Please call Mary Linda Todd in advance (518-486-4858) if you have questions about the changes or the form. The deadline for budget amendments for all grants is January 31, 2008.
The State Library now links to the new FS-10-F Short form for the Final Expenditure Report (Word or .PDF), but the three copies (at least one with original signature) must be submitted DIRECTLY to Library Development by April 30, 2008. The System/Library Director/District Superintendent's signature must appear on page 2 of the form as the Chief Administrator's Certification. Please be sure to fill in the Agency Name, Agency (SED) Code, Project Number, Contract Number and approved budget total in the boxes on page 2.
A final narrative report on the overall project will be due on April 30, 2008. This report form will be posted here.
