This form must be filled out completely. Three copies with three original signatures in blue ink must be received in the New York State Library's Division of Library Development Office by 5:00 p.m., Friday, December 7, 2007, along with your application. Below are some instructions for completing the form. Please contact Lorraine Deitz with any additional questions.
Only costs that you are requesting grant funds for should be included on the FS-20 form.
A copy of any contracts that are necessary for your project must be submitted to this office. Please send the FS-20 (3 copies) with your application by the deadline date. We cannot release any funds to you until we receive this form. The FS-20 form, when approved, will release the first payment of the grant award. This form does not replace the Discretionary Grant budget forms, on yellow paper, which are part of your packet; you will have to return the yellow budget forms when your project is completed in order to receive the final payment. If your project is approved, you will receive an approved signed copy of the FS-20 form that you submitted to the Conservation Office from the Categorical Aid Management Office. This copy of the FS-20 form signifies that payment will be forthcoming in approximately four weeks.
