Application Checklist

Member libraries must submit four copies of their project applications and required attachments directly to the public library system by the local deadlines established by the system. Please contact your public library system for more information about local deadlines and the system's application review and evaluation process. Member libraries should not submit applications directly to the State Library.

Four copies of each application and required attachments must be submitted by the public library system to the New York State Library/Division of Library Development by Friday, September 11, 2009.  No exceptions will be made.

  • Attachments (see Instructions)
    • Construction Project Narrative
    • Project Cost Estimate (Contractor Quote/Bid Documents)
    • Certification of Available Funds to Finance Project
    • Certificate of required minimum lease/legal agreement and project approval from building owner
    • Office of Facilities Planning approval (if building is owned by school district and project over $10,000)
    • SHPO Approval Documentation (if building is more than 50 years old or in an historic district)
  • Short Environmental Assessment Form; in MS Word | .PDF
