'CITY' may be blank if the certificate was issued before our records were computerized.

Troubleshooting:

Separate hyphenated last names ('smith-williams') with a hyphen. For two-word last names , use a space ('van smith'), but also search with no space ('vansmith') or a hyphen ('van-smith') if no results are returned.
